Over the course of your life, you have put together your home to be how you want it. You have set routines that are comfortable and candid. As you get into your senior years, you may begin to find it difficult to keep up with routines. You may find it more challenging to complete certain tasks or make your way around the community. A helping hand will go a long way. In addition, during the time of recovering from an illness or hospitalization, our homemaker companion and discharge support services will allow you to focus on your recovery while we take care of the rest.

It is our mission at Mindful Caring to improve the quality of life of our clients by providing quality care. We achieve this through an in-home consultation where we gather information to get to know our clients, their preferences, and activities so that together we can develop a personalized care plan. Our companion homemaker service combines warmth, compassion, human interaction, and assistance with daily living activities, to provide you with a safe, independent lifestyle in your home. At Mindful Caring we take pride in what we do to meet the satisfaction of our clients.

Mindful Caring caregivers will help so you can live comfortably at home. More importantly, our caregivers promote healthy living and well-being through the benefits of social interaction, safety supervision and encouragement to participate in mindful activities.

We provide the following services in Mindful Caring Homemaker Companion and Discharge Support:

Homemaking and Companion

In addition to being a friendly and familiar face, our caregivers can help you keep your home just the way you’ve learned to love it. This can include:

  • • Taking out the garbage
    • Preparing and folding laundry
    • Light household chores

  • • Helping with meal preparation
    • Grocery shopping

Wellness

Living well is about more than simply good health. Our caregivers can help foster wellness through our person-centered care approach to your mental, physical, emotional, spiritual, and social health. This includes:

  • • Safety supervision
    • Light exercise encouragement if permitted
    • Cognitive stimulation
    • Meal planning

  • • Reading a book aloud
    • Engaging with hobbies
    • Accompanying you on walks or outings

Health Reminders

Our caregivers understand the importance of protecting you or your loved one’s health. Sometimes all it takes is a gentle reminder to take medication or initiate a game to follow the doctor’s orders. We are here to help with:

  • Medication reminders

  • Managing medical, dental, and optical appointments and health screenings

Socialization Support

It is important for your social, mental, and physical health that you’re able to safely make trips for medical appointments or grocery store runs. Our caregivers will help with transportation needs, such as accompanying you on walks or getting to your appointments as listed below:

  • Grocery shopping

  • Leisure outings

  • Running errands

 Hospital to Home

After a stay at the hospital or other care facility, there is nothing like returning to the comfort of home. A discharge from the hospital or care facility can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. Mindful Caring can help with these tasks while you get the rest you need to recover and regain your strength.

The best time to begin thinking about your discharge from the hospital is while you’re still in the hospital. As you prepare to be discharged after a hospitalization, the thought of recuperating at home may be overwhelming. With our discharge support services, we can relieve your stress and organize your homecoming in a safe way. We will communicate with your family and health care providers to notify them of any changes in condition, being mindful of your individualized care needs. We’re here to make living at home easier, so you can save your energy for recovery while we complete the following for you: 

  • coordinate your discharge care.

  • transport you to your home and ensure your safety and comfort

  • schedule your follow-up appointments

  • organize pickup and delivery of your medications.

Questions to ask healthcare providers

  1. Do I need any specialty care or services after my discharge from the hospital?

  2. Communication and coordination between the hospital and family or the next facility are key. Ask who is your hospital care coordinator for this process to be arranged.

  3. What medication schedule should I follow at home?

  4. Are there changes made to my home medications or are there new medications prescribed for me?

  5. What medication side effects should I look for to report to my doctor?

  6. Do I have activity limitations after leaving the hospital?

  7. Do I have any dietary restrictions or nutritional requirements?

  8. What medical equipment will I need at home?

  9. Do I have follow-up medical appointments, and when are those?
